Etiologic Review of Ileal Perforation

Abstract
|

|
Over a period of 5 rears and 8 months, from November 1956 to June 1962, 15 patients with Peritonitis caused from Perforation of the terminal deal lesion were observed at t e Pusan National University Hospital. Cases due to cancer and mechanical causes were not included. The 94 well studied cases were selected out of these 115 cases. In an attempt to discover the etiology of the various cases of deal perforation macroscopical and histological findings at the site a of the lesions and regional lymphnodes were not when typhoid and serological examination an bacteriological examination of pus, urine, stool, and material from the site of the lesions were made and clinical features were also taken into accout.
Significance of criteria
1) We regarded a case as typhoid when typhoid bacilli were discovered from any specimens in blood, urine, pus, stool, or affected tissue in patients with peritonitis due to heal perforation.
2) A case was regarded as quasi-typhoid when the Widal titer alone was higher than 1 : 320. The Widal test was performed more than once in every case.
3) We regarded also as quasi-typhoid when any case in which typhoid macropha e was found in specimens from deal lesions and regional mesenteric lymphnodes.
4) We regarded as definite typhoid when above mentioned both criteria(2) & ( ) were proved positive.
Classification of the results of the studies on the peritonitis due to perforation of the deal lesions can be made as follows.
Group I. Difinite Typhoid 27 cases.
Subgroup I Includes the cases in which typhoid bacilli were found, the tite in the Widal test was. higher than and 1 : 320typhoid macrophage was identified in the lesions 2
Subgroup II includes the cases in which the titer in the Widal test was higher than 1 : 320
and typhoid macrophage was found in the lesions 21 cases.
Subgroup III includes the cases in which_typhoid bacilli were found and typhoid macrophage was found in the lesions 4 cases.
Group II. Quasi-typhoid 63 cases. Only typhoid macrophage was found in the lesions. Only Widal test is positive
higher than. 320 : 1 L 2 cases.
Group Ill. Conditions other than typhoid 4 cases.
Subgroup L Tuberculosis 2 cases.
Subgroup II. Crohn´s disease 1 cases.
Subgroup III. Acute Pseudomembranous ileitis, 1 cases.
